You can now submit all documents electronically!

We are pleased to announce that, effective immediately, you can now submit all types of documents and medical credentials electronically. You are encouraged to send your documents through email at [email protected].

Note:

  1. Candidates who have already submitted electronic copies of Certified Identity Confirmation forms and certified identification documents are no longer required to send the original paper copies. Please note that online certifications cannot be accepted; your documents must be certified in person by a certifying official, and you must sign the form in their presence.
  2. Candidates who have already submitted paper copies of any documents or credentials are not required to submit them again electronically.

Please send us copies of your documents only once (by email, fax, mail or courier). Sending the same documents more than once may cause delays in the processing of your request.
