Helping improve quality of care

MCC 360 is a national program that gathers feedback from a doctor’s patients, colleagues, and co-workers to let them know how they are doing and where they can improve.

MCC 360 evaluates how well a physician communicates and works with others, and how they perform as a health care professional. If you have been asked to complete a survey, it is because they are committed to improving patient care. Your responses can help them achieve that.

For patients

Your patient perspective is valuable. By completing this survey, you will help your doctor reach their goal of improving their practice.

It is your choice whether to take this survey. If you do, know that all your answers will be anonymous. They will be grouped with answers from other patients and then sent to the doctor in a report. Your doctor will not know how you responded. None of the survey questions are about your personal health—only about the quality of care you received.

To complete the survey:

  1. Type the link that your doctor has given you (for example, dr360.ca/XYZ123/en) into the address bar of your web browser (see below). This link is unique to your doctor.

  1. Answer each question in the available field.
  2. Click “Done” at the bottom of the page.

If you get a message that says the link is no longer available, your doctor may have collected enough surveys from other patients to complete the requirements for their MCC 360. Please send an email to [email protected] with the online patient link and the name of your doctor so we can confirm if their MCC 360 evaluation is still active.

Can I complete the survey if I have only seen the doctor once?

The patient surveys are designed so that you can provide feedback even if you have had one interaction with the doctor. Should a question not be relevant to your visit, you may answer “Unable to Assess.”

Are paper surveys available?

Some doctors may have paper copies of the survey. If you would prefer to complete a paper survey, please contact your doctor directly to see if this is an option.

Where do I mail completed paper surveys?

Your doctor was provided with individual envelopes to distribute to their patients along with the survey. You can place the completed survey sealed inside the envelope and return it to your doctor. If you prefer to mail the survey directly to us, our mailing address is:

For colleagues and co-workers

As a health care professional, you are uniquely qualified to provide valuable feedback to a physician you interact with—even if you think you do not know them well. The more feedback they are able to collect, the more accurate their overall assessment will be.

As mentioned in the email you would have received from [email protected], the program wants your honest perspective on how the participating physician performs in three roles as identified in the CanMEDS framework: communicator, collaborator, and professional.

By completing this quick survey, you will help the physician direct their continued professional development and improve the quality of care that they provide.

It’s your choice whether to take this survey. If you are not sure, we encourage you to open the survey and review it before you decide. If you still do not wish to participate, please contact us.

If you choose to participate, know that all your answers will be anonymous. They will be grouped with answers from other colleagues or co-workers and then sent to the physician in a report.

To access the survey:

  1. Click the link in the email from [email protected]; the survey will open in your web browser.
  2. Answer each question honestly in the available field. If you think that you cannot properly assess any of the survey statements, you may select “Unable to Assess.”
  3. Click “Done” at the bottom of the page.

Why was I asked to provide feedback?

We are not made aware of the nature in which the identified co-workers and colleagues share work and patient care with the assessed physician. Completion of the survey is confidential and at the sole discretion of those invited to participate.
