MCC 360 survey system maintenance on July 3, 2020

Please note that MCC 360 surveys (whose delivery is supported by SurveyMonkey) will be temporarily unavailable between 11 a.m. and 11 p.m., Eastern Time (ET) on Friday, July 3, 2020 due to system maintenance. This means that you will not be able to complete your self-assessment, and that your colleagues, co-workers and patients will not be able to complete surveys on your behalf during this time. However, you may continue to access your MCC 360 account.
