Replacing your LMCC documents
How to request replacements of your LMCC Testamur and/or Certificate of Registration.
If you are replacing your documents because your name has changed, follow the instructions on how to request a name change for your LMCC documents.
If you need to replace your LMCC documents for any other reason, please submit a request through your physiciansapply.ca account.
After we receive your request, the service fee for replacing the LMCC testamur and certificate of registration will be applied to your physiciansapply.ca account. Once payment is received, we will begin to process your request.
Please note that LMCC document replacements due to name changes are issued as “Duplicates of Original as Amended”, whereas replacements for reasons other than name changes are issued as “Duplicates.”
